Description: Inpatient and outpatient services are provided to meet the medical, surgical, and psychiatric needs of eligible veterans. Other services include access to Defense and Veterans Brain Injury Centers, provision of durable medical equipment, support groups, respite, personal care, and chore services, adult family home placement, and supportive housing options. Supplemental services include coordination with mental Health and chemical dependency programs. This Program is based on the Program of All-Inclusive Care for the Elderly (PACE) care model.
